Four major deviations in the development of modern medicine

Reporter: What do you think of the relationship between modern medicine and culture?

Fan Daiming: To answer this question, we must first realize the dilemma faced by modern medicine-modern medicine has greatly improved the average life span of human beings, but the question of medicine has never been so fierce as it is today.

As Roy Porter, a professor of medical history at Cambridge University, wrote: People have never lived so long and lived so healthily, and medicine has never achieved so much. However, paradoxically, medicine has never aroused people’s strong doubts and dissatisfaction as it is today.

In fact, there are four deviations in the development of modern medicine: first, medical research blindly develops to technology and penetrates to micro-level, which leads to over-differentiation of specialties, over-detailed planning of specialties and fragmentation of medical knowledge, that is, O2F1;; Second, modern medicine has become waiting medicine; Third, modern medicine has become antagonistic medicine; The fourth is medical alienation, which takes some natural processes of life and some natural changes of the body as diseases for excessive intervention.

Reporter: As for O2F1, you have elaborated it in detail when you were interviewed by us.

Fan Daiming: Yes. This situation is at least a problem caused by introducing simple research methods into medicine, that is, simply introducing science and technology into complex and variable human health care. Doctors are increasingly confined to narrow professional fields, and even can only see a certain disease, resulting in more and more drugs, but the curative effect is getting worse. I will focus on the last three biases.

Modern medicine has become waiting medicine. Modern medicine regards people’s health status as a linear process, from no illness to death due to illness. What is disease? It is to draw a line artificially according to some indicators, and when you cross this line, you are sick, and the doctor will treat you; If you don’t cross this line, the doctor won’t care.

Take stroke as an example. A person is a "good" person without a stroke, and a sudden stroke one day becomes a patient. After the onset of stroke, treatment can hardly play much role, but the money earned by patients in their lifetime is likely to be used up in the last few days.

Only at the end of the disease and the last few days of life, the curative effect is bound to be limited, and it will bring great pain to patients. This is waiting for medicine. If we work harder before we get sick, will the result be different? The "prevention of disease" advocated by Chinese medicine should give enough enlightenment to modern medicine.

Modern medicine has become antagonistic medicine. In the past, infectious diseases caused by foreign sources and a single cause were the greatest threat to human health, so it is understandable to "confront" all diseases as "enemies". However, at present, chronic diseases have become the biggest threat to human health, which is a problem in the internal balance adjustment of human body. If we still adopt the thinking of "confrontation", we are "confronting" ourselves, and we may not cure the "disease" but hurt other organs.

This "confrontational" thinking is also deeply influenced by culture. Modern medicine represented by western medicine originates from nomadic culture, and the survival rule of nomadic culture is "you die and I live"; In China’s traditional culture with thousands of years of farming society as the background, "harmony" and "you live and I live" are the mainstream. Therefore, treating cancer in traditional Chinese medicine is "treating cancer without seeing it" and how to better "survive with cancer".

There has been alienation in medicine. It is too serious to treat some natural processes of life and some natural changes of the body as diseases for excessive intervention.

For example, it is a natural process for a mother to give birth to a baby. Unbalanced medical culture

Reporter: What will the imbalance between medical development and culture, or the lack of culture in medical development, bring to medicine?

Fan Daiming: There are at least three consequences. First, with the help of science and technology, medical research on human body structure and even function has been advanced and thorough, reaching directly to genes, but medical knowledge of the nature of life is still far from being understood culturally, and this is the most important thing. You know, being sick is not only a physical illness, but also a life illness. The imbalance between medical development and cultural relations, or the lack of cultural medicine, makes modern medicine "out of her wits".

Second, worldwide, human culture (including medical culture) has been formed and developed for thousands of years (such as European culture, Indian culture and China culture), but now we are using a single-domain culture (such as medical ethics culture) with only a few hundred years to monopolize or even replace the global human culture for thousands of years, which seems to be somewhat "inadequate".

Third, compared with the past, the spectrum of contemporary human diseases has undergone fundamental changes, such as various endogenous senile diseases and chronic diseases, with multiple causes and multiple targets; In history, human life span has never been as long as it is now, and the biggest threat to health is often infectious diseases caused by external factors and single cause. Modern medicine is still using simple, linear and direct methods to deal with infectious diseases in vitro to deal with the complex, nonlinear, indirect and endogenous chronic diseases of human beings, so it often backfires.

From being out of her wits, to being unable to do what she wants, and then to being counterproductive, won’t this medical culture change its profession? We must make great efforts to reshape medical culture. Where medicine develops depends on what kind of medical culture to lead.

Reporter: Some people say that science and technology and humanities are the two wings of the progress of human society, and they are indispensable, otherwise they will deviate from the direction. It seems that the same is true for human health, medicine and culture.

Fan Daiming: Improper handling of the relationship between medicine and culture is not only bad for patients, but also harmful to doctors. At the memorial service, it is usually said that "the comrade died because of illness", but it is not said that "he died because of illness". In the event of an emergency, the seriously injured are sent to the hospital for rescue, and when notified, they will say "death due to injury", but they will not say "death due to injury". Also, "as long as there is one percent hope, we must use 100 percent efforts to rescue", which is right from the humanitarian point of view, but is it completely right in medical practice? This is a waste of money for some patients, a waste of effort for doctors, and a loss for society.

If "ineffective treatment", "ineffective rescue" and "medical pursuit of 100% success" have become "routine", isn’t it an excessive accusation and injury for doctors? This is also a cultural problem!

Cases of hurting doctors are endless, which is a more extreme phenomenon. Let those who hurt doctors be punished, even sentenced to death, so that the problem can be solved fundamentally and completely? No! We must think from the root of medicine, culture and values, so that we can embark on the right path.

"Four Persistences" Reshape Medical Culture

Reporter: In your opinion, what should be included in dealing with the relationship between medicine and culture?

Fan Daiming: First, we must adhere to the humanity of medicine.

Humanism is the highest realm of culture in the study of human nature, and its function is to ensure the safety, importance and dignity of life. There are two basic requirements of human nature: one is to pursue happiness, the other is to pursue immortality and hope to live forever. In fact, it is impossible for an organism to be immortal. People are the same, flowers bloom and flowers fall, the tide rises and falls, and everything is the same, which is inevitable in nature.

But the problem is that the single-domain medical ethics culture does not recognize that people will die, so they use technology to intervene in death. So there are three problems: first, the time of death has changed from unknown to known. Second, the place of death was moved from home to the ward. We used to take medicine home for illness, but now we send patients directly to the hospital to save their lives, even if we know there is little hope. Doctors in ICU and rescue room tried their best to rescue them, and family members outside tried their best to pay the money. It is likely that when the patient finally left, the family members failed to shake hands and say goodbye. The third is from natural death to technological death. An old expert in our hospital left when he was 92 years old. In the last two years, he became a vegetable, and his soul had already left this world. We still worked hard on his body and relied on a ventilator to maintain it. There are many such people, who have left this world although they are lying in hospital beds. They are covered with tubes but don’t know it. It can be said that they have no dignity at all. The doctor is looking at whether the pipe is impassable. All-pass is alive, no-pass is dead, and part-pass is part-live. Is that right? Isn’t this culture going to change professions?

In the past, both Chinese and western sacrificial customs fully considered the sanctity and dignity of life. At present, many hospitals offer hospice care, which fully considers the feelings of family members and the hospice care of medical staff for the dead. In fact, this is also a reshaping of medical culture.

Second, we must adhere to the integrity of the human body.

We should respect life and respect it. Life exists as a whole. With the infinite division, all the parts finally exist, but what is lost is life. Conversely, the sum of all parts is not equal to a whole, because the whole of medicine must have life. A whole with life is called a whole; There is no whole life, we call it a corpse.

The existence of life must depend on the whole existence. An elephant, we know at a glance, the blind can only rely on touch. When a blind man touches an elephant’s leg, he can tell that it is an elephant, but can he still recognize it if he touches cells or even molecules? If we go to the atomic and quantum level, everything is indistinguishable, can we still identify life forms? Therefore, medicine should not be too microscopic.

Einstein has long said that science pursues clarity, accuracy and purity at the expense of integrity. Similarly, the "accuracy" we blindly pursue in medicine is at the expense of life, which is also a culture and a culture with problems.

Third, we must persist in the complexity of life.

Life has great complexity, and the local and instantaneous research results can not explain the process and essence of life, nor can they be a panacea to save lives.

We know life now, which is too simple, but in fact it is extremely complicated. The existence of life, first of all, is the role of natural forces. Otherwise, how will life be spent in millions of years or even longer without medicine? This natural force is manifested in the ability of life itself to resist diseases and harm; Second, different organs can coordinate with each other and have self-consistency ability; Third, after a certain part of the body is damaged, it can compensate for growth and has the ability of self-repair; Fourth, metabolic ability, life and physical ability blend with the outside world, and absorption of order can discharge disorderly energy; Fifth, self-balance ability, such as water and electrolyte balance, white blood cell level balance, heat balance and so on; Sixth, the ability of self-protection, which is manifested in immunity, vomiting and excretion after eating bad things; Seventh, the reaction of spirit and consciousness to the above six abilities.

There is a saying that the "three magic weapons" for doctors to treat diseases are language, medicine and scalpel. Among them, drugs and scalpels are necessary, and the power of language is the greatest. There is a famous saying in the west, "To cure sometimes, to relief oft, To comfort always". This sentence means that for patients, soothing and comforting are the most important, and healing only accounts for a small part. In fact, even if the patient is cured, in the final analysis, it is the patient’s own internal natural forces that have played a role. The doctor just helped them outside, and never take advantage of it.

Therefore, medical intervention in life and health should first ensure that the natural forces of life play a role. If medical intervention exceeds or even replaces this ability, it is called overtreatment. From the development practice of modern medicine, there are fewer and fewer medical deficiencies, but more and more medical excesses.

Fourth, we should insist on the authenticity of the research.

At present, many medical researches are not true, but only one way to the end, from macro to micro, or from structure to small structure, from long time to short time. We must go back and return to the macro level, which is what we call reverse medical research. Only when the way back is clear and a circle is formed, it is called closed-loop research, and the correct results can be obtained. Historically, many great achievements in natural sciences, especially medicine, were obtained by reverse research.

The fundamental way to change the existing clinical thinking and improve the shortcomings of evidence-based medicine in medical research is reverse thinking. Reverse thinking involves many aspects, including real-world research. Real-world research may also have its own problems. If evidence-based medicine is combined with real-world research, especially reverse research on evidence-based medicine, we will be much more likely to get correct results.

Of course, four persistences are the necessary conditions for reshaping medical culture, and it is far from enough to be satisfied with these four aspects. I hope more people can join in and lay a more solid foundation for reshaping medical culture.

Learn from the sages 2,500 years ago.

Reporter: How to choose the starting point to reshape medical culture?

Fan Daiming: Some people say that to solve many global problems today, we should go back to ancient China 2,500 years ago and learn from the wisdom of Confucius. Confucius lived in the Spring and Autumn Period and the Warring States Period. At that time, there were "three religions and nine streams" and "hundred schools of thought". A large number of outstanding thinkers, represented by Confucius, looked forward to 2500 years and thought about 2500 years later, which set up an ideological monument for Chinese civilization and even world civilization that is beyond the reach of today.

So, where is the cultural essence of the Chinese nation for 5000 years? In my opinion, it is a people-oriented holistic view and an integrated view of the unity of man and nature. This is where the roots and ideological context of holistic integrated medicine come from. On the remolding of medical culture, the organic integration of holism of traditional Chinese medicine and reductionism of western medicine should be the only way.

In China’s traditional culture, everything in the world is divided into five elements: Yin and Yang, which are rooted in each other, and the five elements interact with each other. On this basis, Chinese medicine puts forward that essence is substance, which can be detected by modern medical means, such as hemoglobin, blood lipid, etc., but qi and spirit instruments can not be detected, but Chinese medicine doctors can perceive their state level by looking, hearing and asking. This is the common expression of structure and function, and it is the understanding of medicine by ancient Chinese medicine. Even in today’s advanced science and technology, Chinese medicine still shows strong vitality and good curative effect, even far beyond the cognitive level and curative effect of modern medicine in some aspects, which is worth learning and learning from modernmedicine with an open mind.

In the development history of traditional Chinese medicine, Taoism, Buddhism and Confucianism have contributed a lot, and the reconstruction of modern medical culture can gain important enlightenment from it. For example, Buddhists emphasize the cultivation of the mind, emphasizing a "clean" word and demanding that the mind be free from distractions. It is not easy to do this. There are always many distractions in my heart. Can I be healthy? If you want to calculate others every day, can you be healthy? Another example is that Taoists stress health preservation and emphasize a "quiet" word. Mentality is always manic, can you calm down and nourish your heart? Therefore, it is necessary to be calm in order to ensure health. For another example, Confucianism stresses governing the country, managing the world and helping the people, and emphasizes the word "respect", so as to respect nature, society and etiquette.

Science itself has no purpose, and good use can benefit mankind, such as nuclear power technology; If it is not used well, it can also be used to kill people, such as nuclear weapons. Whether it is used well or not needs the guidance of culture. The same is true for medicine. The profound traditional culture of China, whose holism and integration view are integrated with the reductionism of western medicine, will form a new medical culture. Only by forming an integrated health service system, including an integrated medical education system, a medical research system, a medical service system, a medical prevention system and a medical management system, can we lead the new direction of medical development and go faster and further in implementing the healthy China strategy and caring for human health.

Macroerythrocytes

It is more common in β -lipoprotein deficiency (acanthocyte can reach 80%), and can be seen after splenectomy, alcoholic liver disease, uremia and so on.

Blunt sawtooth red blood cell

That is, shrunk red blood cells, the length, width and spacing of cell surface protrusions are the same (pay attention to distinguish them from blunt sawtooth red blood cells). More common in hypertonic environment or improper production.

cabot ring

Purple-red thin coil structure appearing in red blood cells, sometimes wound in a figure of eight. Its nature is not clear at present, and it is thought that it may be the residue of nuclear membrane or spindle or the product of lipoprotein denaturation in cytoplasm. Common in patients with megaloblastic anemia, hemolytic anemia, leukemia or lead poisoning.

Mouth red blood cell

The pale area in the center of red blood cells is a narrow crack, the center of the crack is narrower than both ends, and the edge of the crack is clear, similar to a slightly open fish mouth. A small amount of oral red blood cells can also be seen in normal people’s peripheral blood, which is generally 3%-4%. If it exceeds 4%, it can be considered abnormal. When it increases, it is more common in hereditary oral polycythemia, which can be seen in DIC, alcoholism, hemolytic anemia and liver disease.

Helmet red blood cell

The shape is irregular, with 2 to 3 sharp corners in helmet shape, which is more common in disseminated intravascular coagulation, microangiopathic hemolytic anemia, cancer metastasis and so on.

Teardrop red blood cell

Red blood cells are teardrop-shaped, pear-shaped, cudgel-shaped, etc., which are most common in myelofibrosis and hemolytic anemia.

schizocyte

The cells are irregular in shape, different in size, and have asymmetric pointed protrusions, which are red blood cell fragments or incomplete red blood cells. Usually irregular triangle, thorn or helmet shape. A small amount of erythrocytes can also be seen in normal people’s peripheral blood, which is generally less than 2%. If it exceeds 2%, it can be considered abnormal. It is a fragment of red blood cells caused by mechanical injury or endogenous abnormality, which is mainly found in DIC, microangiopathic hemolytic anemia, megaloblastic anemia, severe burn and so on.

Money-shaped red blood cells

The arrangement of red blood cells, like copper coins strung together, indicates that the blood viscosity is high, and abnormal proteins appear in plasma, which reduces the negative charge of red blood cells, changes the plasma charge balance, and reduces the repulsive force between red blood cells, thus facilitating the formation of money-like aggregation and accelerating the erythrocyte sedimentation rate. More common in multiple myeloma, hyperglobulinemia, hyperfibrinemia and so on.

The lightly stained area in the center of red blood cells disappeared, and the whole cells were deeply and evenly colored. Under normal circumstances, a small amount of spherical red blood cells can also be seen in peripheral blood, generally less than 3%. The increase in the proportion of spherical red blood cells is more common in hereditary spherocytosis (often more than 25%, sometimes reaching 100%), autoimmune hemolytic anemia, abnormal hemoglobinopathy, liver cirrhosis, hypersplenism, chronic infectious diseases, globin body hemolysis, neonatal hemolysis and hemolytic diseases caused by erythrocyte enzyme deficiency.

Polychromatic erythrocytes

The whole red blood cell or a part of its cytoplasm is grayish blue (due to the coexistence of intracellular RNA and hemoglobin), which is called polychromatic red blood cell. Under normal circumstances, a small amount of polychromatic erythrocytes can also be seen in peripheral blood, which is generally less than 1.5%. The increase of polychromatic erythrocyte ratio is more common in hemolytic anemia or acute hemorrhagic anemia, which indicates that bone marrow proliferative ability is active.

Basophilic stippling erythrocytes

Small blue particles with different sizes and scattered distribution can be seen in red blood cells, which is a relatively naive red blood cell. It may be due to the degeneration, aggregation and precipitation of basophils remaining in the cytoplasm after heavy metal damage to cells and the formation of particles. Under normal circumstances, basophilic spotted red blood cells (about 1/10000) are rarely seen in peripheral blood, but they are more common in heavy metal poisoning (such as lead, mercury, bismuth, etc.).

Howell-Jolly corpuscles

Red blood cells contain 1-2um purplish red round corpuscles with different sizes and numbers. This corpuscle may be the nuclear chromatin remaining during the division of young red blood cells. Common in hemolytic anemia, megaloblastic anemia, splenectomy, erythroleukemia or other proliferative anemia.

Oval red blood cell

Red blood cells are oval or rod-shaped, with a length of 3-4 times greater than the width, a maximum diameter of 12.5μm and a transverse diameter of 2.5 μ m.. This kind of red blood cell is placed in hypertonic, isotonic and hypotonic solution or normal human serum, and its oval shape remains unchanged, but neither young red blood cell nor reticulocyte is oval. Under normal circumstances, oval red blood cells are rarely seen in peripheral blood (about 1%, not more than 15%). In the blood smear of patients with hereditary ovalocytosis, the proportion of such red blood cells can reach 25% or even as high as 75%, and the proportion of oval red blood cells can also increase in thalassemia, megaloblastic anemia, myelosclerosis and iron deficiency anemia.

Nucleated red blood cell

Under normal circumstances, nucleated red blood cells can not be seen in adult peripheral blood, but a small number of nucleated red blood cells can be seen in neonatal peripheral blood within one week of birth. When nucleated red blood cells appear in adult peripheral blood, they can be seen in various proliferative anemia (hemolytic anemia, acute hemorrhagic anemia, megaloblastic anemia and severe hypopigmentation anemia can also appear), malignant tumor or metastasis of hematopoietic system, splenectomy and severe hypoxia.

Crescent red blood cell

Lecture | Du Weisheng, Research Librarian of the National Library: The Binding of Ancient Books in China

Mr. Du Weisheng is a research librarian of the National Library of China, a representative inheritor of national intangible heritage projects, and has been engaged in the restoration of ancient books in the National Library of China for more than 40 years. He is familiar with all kinds of binding forms of ancient books in China, not only mastering the repair techniques of various binding forms of ancient books in China and the mounting techniques of traditional calligraphy and painting in China, but also dealing with all kinds of difficult problems in the process of repairing ancient books. At the same time, he is familiar with the binding and repair techniques of modern hardcover books and paperback books, and also familiar with the binding and repair techniques of ancient hardcover books in Europe. The ancient books he repaired include hundreds of Dunhuang suicide notes from the Southern and Northern Dynasties to the end of the Tang Dynasty, and thousands of rare ancient books such as Wenyuan Yinghua in the Song Dynasty, Yongle Dadian in the Ming Dynasty.

Recently, Du Weisheng was a guest at the 13th lecture of "History of Calligraphy and History of Books" in Peking University Institute of Literature and Art, with the theme of "Paper and Ancient Books in China", which brought us a new understanding of the whole civilization progress system in China with paper as the carrier from the perspective of materials and technological process. Book carrier before the invention of paper

Du Weisheng starts with the book carrier before the invention of paper. At this stage, people mainly write on bamboo slips, wooden slips and silk books, and bamboo and silk books are the earliest known formal books in ancient China. Simplified books appeared in Shang and Zhou Dynasties. In the article Shangshu Doshi, the Duke of Zhou warned the adherents of Yin in the early years of the Western Zhou Dynasty: "Only you know, only your ancestors, there are books and codes." A "book" is a book made up of letters. Both Oracle Bone Inscriptions and Jin Wen have the word "book", which shows that there were already books compiled into volumes at that time.

There are two types of compilation of simplified books: first compilation and then writing, and first writing and then editing. Most of the simplified books compiled first and then written are official documents handed down by Confucian classics or classics. Books written first and then compiled are more common in bookkeeping documents. The differences in the use of bamboo slips and wooden slips are generally written on bamboo slips when there are many words, and on board slips if there are fewer words. In the Book of Rites, it is said: "More than 100 books are written in the strategy, not as good as 100 books." The so-called "strategy" and "square", as Zheng Xuan said in the Eastern Han Dynasty, "strategy is simple; Party, board also ". What we are talking about is the usage of bamboo slips and board slips, where more than 100 words are written on bamboo slips and less than 100 words are written on board slips. Now, the words "celebrity’s handwriting" and "letters" are all derived from this.

Silk books appeared after bamboo and wooden slips, and some silk books were marked with column lines. The two thick lines on the upper and lower sides actually symbolized the string that connected bamboo slips. The thin ones below actually symbolize the gaps between bamboo slips. Now it’s called hangge. In fact, these lines are the legacy of bamboo slips.

The binding forms of Jane Eyre and Silk Book are: first, all the leaves of the book are connected into a long strip, and the thicker paper is directly rolled up like Jane Eyre, while the thinner paper is imitated by silk book with a small stick of important official, and then rolled up, which is called "rolling".

Bamboo tubes and lacquered wooden cases: Silk books can be rolled up with scrolls or folded and put in fittings. Common containers include bamboo barns (small boxes woven from bamboo strips) and lacquer wood boxes.

Taking Mawangdui silk as an example, Du Weisheng mentioned that many people attach importance to the writing of silk, but its binding form is actually more valuable. It was arranged leaf by leaf and put in a box. In fact, this form is no longer a package, which is the beginning of the leaf system of ancient books in China. Except that the leaves are not bound together, this book already has all the elements of a leaf book.

The invention of paper and the development of binding mode

"Paper is a flat and uniform sheet-like fiber product, and it is the carrier of writing and printing. ….. "When talking about the definition of modern paper, Du Weisheng pointed out that" plant fiber "had to be added to the definition in the early years, but with the spread of chemical fiber paper in the market, the definition of paper also changed. Therefore, paper is a material closely related to the times.

The invention of paper, traditionally speaking, Cai Lun in the Eastern Han Dynasty summarized the previous paper-making technology and improved it, and in the first year of Yuanxing (105) played its improved method. Emperor Jiashan’s ability, Cai Lun’s paper-making method was promoted. From the perspective of technical history, the invention of bamboo paper, that is, bamboo fiber (stalk) papermaking technology, is a milestone event in China’s papermaking history, which provides technical preparation for future generations to make paper from wood.

As for the time when bamboo paper appeared, according to the existing materials, Du Wei pointed out that bamboo paper had not been seen in the Tang Dynasty. Yi Xiaohui, a librarian of the National Library, added that according to the literature, some people think that bamboo paper originated in Sui and Tang Dynasties. But why we can’t see the real thing of bamboo paper in modern times is actually related to the characteristics of bamboo paper raw materials, which have high lignin content and are not resistant to aging.

And why can bamboo paper, which is not very aging-resistant, be preserved after the Song Dynasty? Because there was an innovation in paper technology during this period. Before the Song Dynasty, bamboo was important for life, but it had not yet entered the cultural field. In the Song Dynasty, bamboo paper just caught up with the advent of printing, which required a lot of paper, and traditional leather paper could not meet this demand. The two technologies promote each other and promote the innovative technology of mass production and preservation of bamboo paper.

In addition, the quality, thickness and excellent production of paper directly affect the binding form and service life of books. Before the Song Dynasty, books were mostly written by hand, and the paper was generally thick, mostly between 0.8 and 0.13 mm. At this time, books were mostly rolled. In the Song Dynasty, printing flourished and book production reached an unprecedented peak. At this time, in order to reduce the production cost of books, the book paper is much thinner than that in the Tang Dynasty, generally 0.06-0.08 mm, and the thinnest paper has reached about 0.05 mm.

Because the leaves of the book are thin, if the book is still in the form of roll, it will be difficult to roll it up. In this way, the binding form of books has also changed, from roll to leaf butterfly. Later, in order to adapt to thinner book paper and speed up the binding of books, Bao Beizhuang, thread binding and other binding forms have gradually developed.

The connection between binding forms 

The binding forms of ancient books in China mainly include simple strategy, scroll binding, folding binding, Van Gogh binding, butterfly binding, Bao Beizhuang binding, thread binding and so on. With the change of book-making materials and the development of book-making technology, book binding forms are constantly evolving, which is more conducive to people’s reading and book preservation and protection. When talking about silk fittings, Du Weisheng mentioned that every binding form of China’s books is related.

After the appearance of paper books, it inherited the binding form of simple strategies and silk books, which is called "rolling". Rolling is a common binding form in ancient books in China. At the end of the Song Dynasty, in order to protect books and increase their appreciation, there appeared a mounting form in which the four sides of rolled books were inlaid with silk fabrics such as twill and silk, stretched and calendered, and then added with heaven and earth poles. Mounted pieces that are horizontally displayed become "hand scrolls", and those that are vertically hung for appreciation are called "vertical shafts" or "hanging shafts". Until today, the binding of calligraphy and painting works in China generally still uses this form.

Bookleaf book is the last stage of the development of book binding in ancient China, and it is also the best representative binding art among ancient books in China. The main feature of album books is that many single pages are assembled and bound into a book, which is also a completely different book form from the simple silk and scroll books that are spread and read. It is a revolution in the history of book binding in China.

Books with leaves mainly include Van Gogh’s clip-on, warp-folding, whirlwind or Long Lin’s, sewing, stitching and many other forms. This form of Sanskrit sandwich was first seen in the ancient Indian Buddhist classics written on Bedolo leaves in Sanskrit, and was mainly popular in the Central Plains of Sui and Tang Dynasties after it was introduced to China. The Van Gogh folder folds the scallops with written words in sequence, clamps them with a board from top to bottom, and pierces holes and ropes. In addition, the Van Gogh clip is not written, but engraved, engraved with a pen with a needle. After engraving, it is smeared with red oil or black oil and infiltrated from the place where the pen tip is cut, which becomes a scarlet letter or black word.

Bei Ye Jing in Dai Language (Fan Jia Zhuang)

The warp folding is developed under the influence of Van Gogh’s clip-on. The method is to stick the written and printed leaves together into a long strip, then fold them evenly according to the fixed specifications, and then stick a thick paper on the first and second leaves as the book cover. Because Buddhist and Taoist classics mostly adopt this binding form, it is called "folding". Folding is also a common binding form in ancient books in China, which can often be seen in ancient costume TV series.

Warp folding

In the late Tang Dynasty and the Five Dynasties, there was also a popular binding form between scroll binding and leaf binding-"whirlwind binding". According to the literature records and the physical objects in the existing Dunhuang suicide note, the adhered leaves are repeatedly folded from the end of the book to the front of the book according to certain specifications, and the front of the book is slightly longer as the backing paper, and the left folding mouth is gathered and fixed, clamped with a stick, punched and threaded; Or a single book leaf written on both sides is collected in sequence, put on a piece of base paper, and the left side is directly bound with thread; The leaves of the book are arranged in an orderly way during exhibition and reading, and rolled up with the collected side as the axis during collection. Whirlwind has basically possessed some characteristics of book-leaf system. Books can be "leafed through" when they are being read, and they are "making a whirlwind leaf paper" when they are rolled up. It can be said that "whirlwind" is an improvement of rolled books.

Long Lin Zhuang

In the Tang Dynasty, "stitching and binding" was also popular for a short time. The appearance of "stitching and binding" is close to that of hardcover books now, and the biggest difference is the bare spine. When talking about the binding technology of "stitching" books, which pioneered modern hardcover books, Du Weisheng put forward a questioning conjecture against the statement that "hardcover books came from the west": in fact, western hardcover books were stitched from China, spread to Central Asia and then spread to Europe. The basis is that the binding mode of western books is from volume to hardcover, with a vacuum in the middle, while the binding mode of ancient books in China is consistent with each other.

The closest thing to the appearance of modern paperback books is "Bao Beizhuang", and the most typical book by Bao Beizhuang is "Si Ku Quan Shu". Bao Beizhuang’s way of folding leaves is the opposite of butterfly dress. Fold the page without words and the page with words outward, and then bind it. The appearance of paperback books is exactly the same as that of today’s paperback books. The difference between paperback books and ancient books on the back is that paperback books are printed on two sides of a book, while Bao Beizhuang ancient books are folded on two sides of a book. There is only one difference.

Sikuquanshu

The origin of thread packing is very early, which existed in the Tang and Five Dynasties. Du Weisheng mentioned that he saw the thread packing in the Dunhuang suicide note in France, and the method of setting the line was basically the same as today’s thread packing. At present, thread-bound books generally have four eyeliners, while the thread-bound book in Dunhuang suicide note has three eyeliners, so one eye is punched up and down, one eye is punched in the middle, and it is sewn with a single thread. Except for the number of thread-bound eyes, the others are exactly the same as the common thread-bound books.

After the middle of the Ming Dynasty, in order to adapt to the frequent reading of books, thread binding became popular. Thread-bound is the same as the folding way of bag back. The spine of the book is exposed, nailed and threaded, and bound into a book. Thread-bound is the most common binding form of ancient books in China after the middle of Ming Dynasty. The significance of the appearance of thread-bound books to ancient books in China is: firstly, it simplifies the binding process, but improves the labor efficiency; Third, it provides technical conditions for large-scale production of books.
